Summary
COVID-19 infection results in alteration of normal hematopoietic process in the bone marrow ultimately leading to changes in the circulating leukocytes. Specifically, COVID-19 causes lymphopenia, in other words a decrease in lymphocytes, especially T-cells subsets. The number of neutrophils, however, increases leading to high neutrophil to lymphocyte ratio which correlates with disease severity. The decrease in lymphocytes is primarily due to the direct infection of lymphocytes by SARS-CoV-2, the virus that causes COVID-19, and dysregulation of hematopoietic pathways. The increase in neutrophils is secondary to the increased inflammation caused by cytokine storm, a dangerous overreaction of the immune system. The changes in leukocytes have been associated with worse prognosis and higher mortality emphasizing the significant deleterious effect of COVID-19 on the immune system.

Children who presented in critical condition and were subsequently diagnosed with cancer had a delayed diagnosis and presented with fevers, emesis, hallucinations, and other symptoms. Despite a historical mean of 2.96 days between new leukemia patients, the hospital did not see any patients with a new leukemia diagnosis for 35 days.
